Physicals & Requirements
This past school year, Bonner & Prendie began utilizing a new platform to collect Sports Physicals, FamilyID.com.
Family ID is an online platform that will streamline our Physical collection process for the 2020-21 school year. All families will need to do is create a family account, plug in family information, then upload a copy (Or picture) of section 6 of the PIAA Physical for each BP student-athlete they are registering. Going forward, we will not be collecting hard copies of Physicals.
Instructions to create your account and upload section 6 are attached. Section 6 of the PIAA Physical is also attached.
All physicals for the 2021-22 school year must be dated after June 1st, 2021.
Physicals need to be submitted prior to our first official practice date: Those dates will be announced later in the summer.

ATTENTION BP CLASS OF 2023!
The Pennsylvania Department of Health requires students to have a physical examination prior to 11th Grade. We encourage parents to turn in documentation based on the 16-year well check because mandatory vaccinations are usually updated at this appointment as well. Any exam dated after July 1, 2019 will be accepted.
Any documentation of a physical exam with medical signature can be accepted (including the PIAA Sports Physical Form). If submitting the PIAA Sports Physical form as documentation of the 11th grade physical requirement, please be sure to make a separate copy for the nurse and attach your student’s current immunization record. If you have not already done so, please have this medical information ready to be turned in at the start of the 2020-2021 school year.
