Fundraising Consent Form

This form is to be filled out by any person or persons that plan on holding a fundraising event or campaign where the main participants and supporters of the event will be Bonner & Prendie families, alumni/ae, and friends. It will require the signature of both the department head/moderator and someone from Institutional Advancement. Please obtain these signatures to proceed with your fundraising event. 

This process is being implemented in an effort to help all of us centralize and streamline our fundraising efforts as well as to help us to be aware of what is going on so that we can lend our support and presence more often and in a more effective way.
